Abstract

Background: Digital transformation in healthcare has emerged as a critical paradigm shift, particularly in hospital management systems across India. The integration of digital technologies in hospital operations has demonstrated significant potential for enhancing operational efficiency, patient care quality, and administrative effectiveness.

Objective: This systematic review aims to comprehensively assess the impact of digital transformation initiatives on hospital management practices, with specific emphasis on Indian healthcare institutions, and to identify key implementation challenges and success factors.

Methods: A systematic literature review was conducted following PRISMA guidelines, searching multiple databases including PubMed, Scopus, IEEE Xplore, and Google Scholar for studies published between 2018-2024. Keywords included "digital transformation," "hospital management," "healthcare digitalization," "electronic health records," and "Indian healthcare." Inclusion criteria encompassed peer-reviewed articles, case studies, and reports focusing on digital transformation in hospital management within the Indian context.

Results: Forty-two studies met the inclusion criteria, representing 156 hospitals across 18 Indian states. The review identified five primary domains of digital transformation impact: Electronic Health Records (EHR) implementation (78% of studies), Hospital Information Systems (HIS) integration (65%), telemedicine adoption (58%), digital payment systems (45%), and artificial intelligence applications (32%). Significant improvements were observed in patient satisfaction scores (mean increase of 23.4%), operational efficiency (19.8% improvement), and cost reduction (average 15.6% decrease in administrative costs).

Conclusions: Digital transformation demonstrates substantial positive impact on hospital management effectiveness in the Indian healthcare context. However, implementation challenges including infrastructure limitations, staff training requirements, and financial constraints remain significant barriers. Strategic planning, stakeholder engagement, and phased implementation approaches emerge as critical success factors.
